PATIENTS
If you are a resident of Carter, Greene, Hancock, Hawkins, Johnson, Sullivan, Washington, and Unicoi in Tennessee, or Lee, Scott, and Wise counties in Virginia, or if you are a full-time college student and have no medical insurance, you may apply for medical care.
The information above applies for dental care as well. In addition, our clinic can serve dental patients on Social Security or disability. Similar to medical patients, if you do not have dental insurance, you may apply for dental care. You may have insurance and still apply. We’ll evaluate the amount of coverage given by your insurance plan to determine if your underinsured.
Click “Online Application” and apply online or print a paper application. Applying online is the usually the fastest route to become a patient as long as you’ve submitted all the proper documentation with your application. Failure to do so will result in delays and even automatic denial. You may also come by our office during clinical hours for a paper application.
PAPER APPLICANTS –
Return the completed application with ALL of the required documents back to our clinic during our designated enrollment time.
9AM TO 4PM, MONDAY – THURSDAY.
All required documents must be included along with your completed application:
1. Photo identification (such as a driver’s license)
2. Recent telephone or utility bill with your name and address on it
3. Social Security card for all individuals applying for services
4. Copy of last year’s income tax return, along with Schedule C (if self employed), W2’s, 1099, etc.
5. Verification letters from any government agencies such as TennCare or the Department of Human Services. Statements providing food stamp amount, Families First amount, etc.
6. Forms or statements from Social Security Administration that verifies benefit amount(s)
7. Child support documentation that verifies amount of child support received
Application will NOT be processed until all documents are received.

Once approved, you’ll receive a call from our office to schedule your first medical and or dental appointment. Please arrive 15 mins early to finish new patient paperwork.
